- Abstract:
- Objective To evaluate the efficacy and safety of a preoperative planning model assisted by a combined application of three-dimensional reconstruction, three dimensional (3D) printing and finite element analysis in periacetabular osteotomy (PAO) for adolescents with developmental dysplasia of the hip (DDH).Methods Retrospective review was performed for the relevant clinical data of 40 DDH children undergoing PAO at Children’s Hospital of Fudan University from January 2024 to September 2025.According to preoperative planning mode, they were assigned into two groups of conventional (n=20) and three-dimensional reconstruction (n=20).In conventional group, preoperative planning was performed with radiographs and computed tomography (CT) images. In three-dimensional reconstruction group,CT-based three-dimensional reconstruction,3D-printed physical models and finite element analysis were employed for assisting preoperative parameter measurement, surgical simulation and preoperative planning.Operative duration, intraoperative volume of blood loss, frequency of intraoperative fluoroscopic exposures,Harris hip score, correction of T?nnis angle, lateral center-edge angle (LCEA) and anterior center-edge angle (ACEA) were compared between two groups. Results The mean operative duration was [(76.09±5.08) vs. (82.12±9.19) min] and frequency of intraoperative fluoroscopic exposures [(12.65±3.38) vs. (16.10±3.85) times] differed significantly between two groups (P<0.05).No statistically significant inter-group difference existed in intraoperative blood loss [(536.15±160.68) vs. (559.85±152.87) mL] (P>0.05).The absolute differences between postoperative angles and preoperatively planned target angles were significantly smaller in three-dimensional reconstruction group for T?nnis angle [1.005(0.530,1.733)° vs. 1.805(0.928,3.755)°] and ACEA [2.355(1.300,7.685)° vs. 4.900(2.808,9.795)°] (P<0.05).No statistically significant inter-group difference existed in absolute difference for LCEA [2.255(1.193,7.118)° vs. 5.425(2.050,8.355)°] (P>0.05).All procedures were completed uneventfully in both groups. There were no PAO-related complications such as neurovascular injury or postoperative osteotomy nonunion. Conclusions Individualized preoperative planning based on three-dimensional reconstruction,3D printing and finite element analysis may improve the accuracy of PAO and optimize the consistency between preoperative planning and surgical implemetation.
