- 目前腹腔镜胆总管囊肿根治术已成为小儿胆总管囊肿外科治疗的标准术式,主要手术并发症包括出血、周围组织损伤、胆漏、术后胆道梗阻、胆石形成、胆管炎、胰石形成、胰腺炎、Roux空肠胆支扭转梗阻、囊肿切除不完全导致癌变等。对胆总管囊肿病理改变认知的不断更新、手术时机的精准把握以及外科技术的改进,可减少腹腔镜胆总管囊肿根治术后并发症的发生,从而最大程度实现微创手术目标。本文基于并发症的发生原因与防治阐述腹腔镜手术治疗小儿胆总管囊肿的新进展。
- Abstract:
- Laparoscopy has become a gold standard treatment of choledochal cysts in children nowadays. The potential postoperative complications include hemorrhage,accidental injury,bile leak,biliary obstruction,biliary stone formation,cholangitis,pancreatic stone formation,pancreatitis,Roux loop obstruction and carcinoma induced by incomplete resection. Deeper understanding of pathophysiology,timing of surgery and improved surgical techniques shall minimize the complications and realize the advanced goal of mini-invasive surgery. This review summarized the latest advances of laparoscopic treatment of choledochal cyst from the perspectives of causes,preventions and counter-measures of complications.
