- 儿童胃扭转罕见,好发于新生儿及婴幼儿。根据扭转病因可分为原发性和继发性,其中继发性占70%;根据扭转轴向又可分为3型。由于儿童胃扭转合并先天畸形的种类多样,临床表现特异性较低,术前及时、准确诊断较为困难,容易导致误诊、延诊。急性发作病例若治疗不及时,易导致胃壁缺血、坏死、穿孔,甚至休克、死亡等严重并发症。随着诊断水平的不断提升,大部分儿童胃扭转可得到及时处理,但治疗方式的选择仍存在争议。本文拟对儿童胃扭转的分型、诊断、治疗选择及预后等进行综述。
- Abstract:
- As a rare condition,gastric volvulus occurs predominantly in neonates and infants.According to different etiologies,it may be classified into primary (about 30%) and secondary (about 70%) types.It is divided into three types based upon the rotation axis.Due to its multiple congenital associated malformations and non-specific clinical presentations,it is rather difficult to make a timely and accurate diagnosis,leading frequently to misdiagnosis and delayed diagnosis.Diagnostic delays of acute gastric volvulus result in such severe complications as gastric wall ischemia,necrosis,perforation,shock and even death in children.With the constant improvements of diagnostic technology,most children can be timely treated.However,a proper selection of surgical intervention has remained controversial.This review summarized the classification,diagnosis,treatments and outcomes of pediatric gastric volvulus.
