摘要:
经皮内镜下胃造瘘术(percutaneous endoscopic gastrostomy,PEG)是一种在内镜引导下经上腹壁进行的胃造瘘方式。PEG由于具有创伤小、手术时间短、恢复快等优势,成为需要长期肠内营养支持患者的首选方法。但该技术在儿科开展的风险和难度相对较大,目前国内鲜有报道。部分儿童因基础疾病复杂、腹腔空间小、肥胖、左肝肥大等干扰因素,使PEG的风险增加,为避免术中损伤周围脏器和血管,常需要腹腔镜辅助。但对于各种原因致无法行腹腔镜辅助,且体表可供穿刺点存在局限的患儿,超声作为一种无创便捷的检查技术,可与PEG的优点相结合,提高PEG的安全性和成功率。浙江大学医学院附属儿童医院对一例既往有多次腹腔手术史的患儿,成功实施超声辅助经皮内镜下胃造瘘术。
