- 随着腔镜技术的普及与发展,胆总管囊肿的临床治疗取得了长足进步,但手术操作细节的把控、少见畸形的识别与处理等问题,仍直接影响最终疗效,亟待临床医师重视。本文结合相关文献及临床操作规范,从治疗时机选择、囊肿切除范围界定、空肠升支长度确定、近端肝总管狭窄矫治、远端共同管内蛋白栓清除、副肝管保护、肝右动脉畸形处置、巨大胆总管囊肿的术式优化、囊肿穿孔的急诊处理、机器人手术系统的临床应用、术后吻合口狭窄的预防及远期随访策略等多个方面展开系统阐述,为临床同道提供参考,共同推动该领域诊疗水平的提升。
- Abstract:
- With greater popularization and rapid development of endoscopic technology,significant advances have been made for managing common bile duct cysts.However,proper control of surgical procedures and accurate identification of rare deformities may directly affect the final efficacy and require urgent solutions.This review focused upon the relevant literature and clinical operative standards to systematically address treatment timing,cyst resection ranging,determining jejunal ascending branch length,correcting proximal common hepatic duct stenosis,removing protein emboli in distal common ducts,preserving accessory hepatic ducts,repairing right hepatic artery malformation,optimizing surgical procedures for giant common hepatic duct cysts,emergency treatment of cyst perforation,clinical application of robotic surgical systems,preventing postoperative anastomotic stenosis and long-term follow-up strategies.It was intended to provide reference for clinical practitioners and jointly enhance the levels of diagnosis and treatment.
