- 目的 探讨介入栓塞术治疗儿童颈动脉海绵窦瘘(carotid-cavernous sinus fistula,CCF)的疗效以及介入治疗策略。 方法 回顾性分析2022年 5月湖南省儿童医院神经外科采取介入栓塞术治疗并获得成功的1例儿童颈动脉海绵窦瘘患儿临床资料。检索1980—2022年PubMed、Embase数据库中相关外文文献,检索词为"carotid-cavernous sinus fistula in children;embolization operation";万方医学网、中国知网及中国生物医学文献服务系统中相关中文文献,检索词为"儿童颈动脉海绵窦瘘、介入栓塞手术"。总结分析本例以及既往文献报道的患儿临床症状、疾病类型、治疗方案及治疗结果。 结果 共检索到相关儿童颈动脉海绵窦瘘文献6篇(共6例患儿),结合本文报道1例,共7例患儿纳入分析,男6例、女1例,年龄1个月21天至9岁2个月(平均年龄5.87岁),均以眼部症状(如突眼、眼球活动障碍、上睑下垂等)就诊,其中创伤性CCF 5例、自发性CCF 2例。均采用介入栓塞术治疗。1例死亡,6例存活。6例存活患儿于1~12个月随访过程中眼睑下垂、突眼等症状消失,眼球活动恢复正常。 结论 儿童颈动脉海绵窦瘘少见,介入栓塞术为威胁生命的颈动脉海绵窦瘘提供了新的救治方法。
- Abstract:
- Objective To explore the efficacy and interventional treatment strategies of embolization for pediatric carotid-cavernous sinus fistula (CCF).Methods A retrospective analysis was conducted on the clinical data of a pediatric patient with CCF who successfully underwent interventional embolization in May 2022 at the Department of Neurosurgery,Hunan Children’s Hospital.Relevant literature from 1980 to 2022 was retrieved from PubMed and Embase using the search term "carotid-cavernous sinus fistula in children; embolization operation" and from Chinese databases including Wanfang Medical Network,CNKI,and the Chinese Biomedical Literature Service System using the search terms "pediatric carotid-cavernous sinus fistula,interventional embolization surgery".The clinical symptoms,disease types,treatment plans,and outcomes of this case and those reported in previous literature were summarized and analyzed.Results 6 relevant articles on pediatric CCF were found,reporting a total of 6 cases.Including the present case,7 cases were analyzed.Of these 7 cases,6 were males and 1 was female,with ages ranging from 1 month and 21 days to 9 years and 2 months,with an average age of 5.87 years.All presented with ocular symptoms such as exophthalmos,ocular motility disorder,and ptosis.5 cases were traumatic CCF,and 2 were spontaneous CCF.All were treated with interventional embolization.One patient died,while 6 survived.During the follow-up period of 1 to 12 months,symptoms such as ptosis and exophthalmos disappeared in the 6 surviving patients,and normal eye movement was restored.Conclusions Carotid-cavernous sinus sinus fistula is rare in children.Interventional embolization provides a new life-saving treatment method for this condition.
