- 肝胆肿瘤手术复杂,手术后存在一定的复发风险,需要不断学习新的理论,应用新的技术,才能提高治疗效果。由于肝脏血供丰富,因此无血手术理念在肝脏外科颇具现实意义。只有深入践行无血手术,才能保证手术安全,而手术安全性也是精准外科的基础。精准手术是通过各种先进的技术手段,以最小创伤侵袭和最大肝脏保护来获取最佳治疗效果。随着肝脏手术技术的提高,研究者们希望追求各种更复杂的超级肝切除。然而,科学客观的决策需要在超级肝切除和终极肝移植之间进行平衡。微创手术是近年来肝脏外科的一个重要方向,已经并将继续深入和广泛开展。当然,无论是手术的复杂化,还是创伤的微小化,追求最佳生存获益的肿瘤学原则是最根本的治疗要求。
- Abstract:
- The surgery of hepatobiliary cancer is rather complicated and dangerous so that new concepts and techniques should be adopted for improving its managements.As for rich blood supply of liver,the concept of bloodless procedure is particularly practical for hepatobliary surgery.Only through such a practice,safety of surgery may be guaranteed for enabling precision surgery.Through various kinds of techniques,precision surgery yields optimal outcomes with the smallest invasion and utmost hepatic protection.The tecnique advancements of hepatectomy have enabled more and more complicated super-hepatectomy.Meanwhile,a scientific and objective decision should be based upon a balance of super hepatetomy and liver transplant.As a vital aspect of liver surgery,mini-invasive procedure should be popularized more widely.Whatever the complication of procedure or minimization of invasion,oncological principle is fundamentally required for obtaining optimal survival benefits.
