摘要:
 目的探讨先天性隐匿阴茎的分型、手术指征和治疗方法。方法回顾性分析2014年1月至2017年12月由首都医科大学附属北京儿童医院收治的60例先天性隐匿阴茎患儿临床资料，年龄3~15岁，平均7.5岁，尿道外口与包皮口在同一条直线上为圆锥型（n=42），尿道外口与包皮口不在同一条直线上为烟斗型(n=18)。其中29例圆锥型予保守治疗，13例圆锥型和18例烟斗型予改良Shiraki术治疗。结果60例患儿中，55例获得随访，26例短狭窄段圆锥型通过保守治疗6个月以上，其中15例完全上翻，11例部分外翻；其中12例年龄>12岁短狭窄段圆锥型患儿的包皮均能完全上翻且没有狭窄环。29例手术治疗者，包皮均能完全上翻，其中26例阴茎体显露满意，3例阴茎体显露欠佳。结论先天性隐匿阴茎应采取个体化治疗，可根据尿道外口与包皮口是否在同一条直线上对隐匿阴茎进行分型，短狭窄段圆锥型通过保守治疗可自愈，长狭窄段圆锥型和烟斗型需手术治疗。
Abstract:
 ObjectiveTo explore the classification and treatment of concealed penis in children.MethodsFrom January 2014 to December 2017,a total of 60 children with concealed penis were reviewed and classified by site relation between urethral orifice and foreskin.The clinical types were pyramid (urethral orifice and foreskin in a straight line,n=42) and pipe (urethral orifice and foreskin not in a straight line,n=18).Pyramid (n=13) and pipe (n=18) types underwent modified Shiraki procedure while pyramid type (n=29) received conservative treatment.ResultsAmong them,55 children were followed up.The glans of 26 pyramid type were treated conservatively while eventual appearance of operated cases was satisfactory (n=26) and minuscule (n=3).ConclusionWith fewer complications and satisfactory cosmetics,different types of pediatric concealed penis may be adequately managed by individualized treatments.
