唇样骨切除术治疗儿童股骨头坏死的短期疗效观察

- 目的观察股骨头坏死唇样骨切除术后患儿的短期临床疗效，探讨儿童股骨头坏死的治疗方法。方法我们收治4例（4髋）股骨头坏死患儿，男、女各2例。均曾因发育性髋关节脱位（DDH）接受保守治疗，后遗留髋关节半脱位伴股骨头坏死，手术治疗中因股骨头畸形使复位困难，遂用刀切除外翻的唇样骨及向外突出的赘骨部分，修整股骨头，达到满意的中心性复位。结果4例患儿均随访2~3年，疼痛均明显减轻或消失；3例无跛行，1例跛行改善；4例术前髋外展仅5°~10°，术后3例恢复至正常，1例获改善。头臼指数（AHI）术后平均达84.3%，CE角术后改善到25°。Sharp角恢复到31.5°。短期疗效评估：3例为优；1例为良。结论唇样骨切除术治疗儿童股骨头坏死短期疗效较好，髋部疼痛减轻，活动范围受限改善显著。
- Abstract:
- Objetive To observe the short term results of treatment of LeggCalvePerthes Disease (LCPD) in children with cheilectomy.MethodsFour Developmental dysplasia of the hip (DDH) patients (4 hips) were included in this study， 2 boys and 2 girls respectively. All 4 patients received expectant treatment， and were left with sequelae of LCPD. The deformed femoral head made it difficult to perform operative reduction. The valgus liplike bone was removed by knife. And the femoral head were mended to get satisfied central reduction.ResultsSatisfied clinical and Xray manifestation were observed post 2~3 years followup. The pain was alleviated or disappeared in 4 hips. No obvious lameness was found in 3 hips， and improvement was found in 1 hip. Before operation the hip extension is only 5 to 10 degrees， after operation 3 hips returned to normality and 1 hip improved partly. AHI reached 84.3% averagely， CE angle improved to 25 degrees， Sharp angle returned to 31.5 degrees. The short term curative effect was evaluated， the 3 hips were excellent，1 hip was good.ConclusionThe cheilectomy is effective in treating LeggCalvePerthes Disease in children. It can alleviate pain in hips and improve range of motion for short periods.
